• Deductible: $0 for Preventive (Type 1); $50 for Basic & Major (Type 2 & 3)

• Preventive (Type 1): 100% coverage

• Basic (Type 2): 80% coverage

• Major (Type 3): 50% coverage

• Orthodontia: 50% coverage, $1,500 lifetime maximum
